Made out in duplicate (no carbon paper required), the original (perforated in four places) is the for the quarterly mailing to church members. The duplicate remains as a permanent church record. Pages are punched to fit Binder for SR-11. The tear-out size of each quarterly report is 3" x 8 1/2".
